HHS-R-0371-19 RESOLUTION -- Recommendation of a First Scope of Work Modification, First Time Extension Request of a Community Development Block Grant Agreement, Project Number CD18-05, with Kenneth Moy DuPage Care Center Changing the Scope of Work to Replacement of Two Water Heaters and Extending the Project Completion Date to April 30, 2020.
B. HHS-R-0372-19 RESOLUTION -- Recommendation for a Second Modification, First Time Extension of a Community Development Block Grant Agreement, Project Number CD18-06 with Midwest Shelter for Homeless Veterans - 433 S. Carlton Rehabilitation – Extending the Project Completion Date to July 31, 2020.

A. DuPage Care Center Requests That Also Require Finance And/Or County Board Approval
1. HHS-P-0288-19 Recommendations for the approval of a contract purchase order to Pulmonary Exchange, LTD dba PEL/VIP Medical Staffing, for the rental of respiratory care equipment for the DuPage Care Center, for the period October 22, 2019 through October 21, 2020, for a total contract not to exceed $32,000.00, per second optional renewal under quote Q17-209-GV.
2. Change Order -- HHS-P-0028A-19 - Amendment to Resolution HHS-P-0028-19, issued to Advacare Systems, to increase the contract in the amount of $18,500.00, for rental of low air loss and bariatric mattresses/beds and low air loss wheelchair cushions (category 1 & 2), taking the contract amount of $70,000.00, resulting in an amended contract total amount not to exceed $88,500.00, an increase of 26.43%. (3701-0001 SERV)
B. DuPage Care Center Requests for Parent Committee Final Approval
Change Order -- Symbria Rehab, Inc., to adopt Medicare Part A new payment methodology for Symbria, Rehab, Inc. for the Therapy and Consulting contract, for the DuPage Care Center. (4090-0001 SERV, no change in contract total)
9. BUDGET TRANSFERS
Budget Transfers -- DuPage Care Center - To transfer funds to allow purchases for laundry machine repair parts, DuPage Care Center transportation van labor and repair parts, kitchen cleaning chemicals, and supplies and laundry chemicals for FY19 - $19,900
10. TRAVEL REQUESTS - OVERNIGHT - REQUIRE COUNTY BOARD APPROVAL
Authorization for Overnight Travel -- Community Services - Senior Services Case Manager to attend the mandatory Comprehensive Care Coordination (CCC) Certification required by the Illinois Department on Aging (IDOA) in Springfield, Illinois, from November 18, 2019 through November 21, 2019. Expenses to include registration, transportation, and per diems for approximate total of $665.29. Senior Services grant funded 5000-1720

A. Consent Item -- JDF Services Inc. dba Brightstar Care of Central DuPage, for providing supplemental staffing (nursing) to the DuPage Care Center, for the period April 10, 2017 through April 9, 2018, decreasing and closing contract in the amount of $345,026.09, contract 2414-0001 SERV expired.
B. Consent Item -- Maxim Staffing Solutions, for providing supplemental staffing (nursing) to the DuPage Care Center, for the period April 10, 2017 through April 9, 2018, decreasing and closing contract in the amount of $115,927.41, contract 2415-0001 SERV expired.
C. Consent Item -- Quality Placement Authority (QPA), for providing supplemental staffing (nursing) to the DuPage Care Center, for the period April 10, 2017 through April 9, 2018, decreasing and closing contract in the amount of $201,373.18, contract 2418-0001 SERV expired.
D. Consent Item -- Washburn Machinery, Inc., for laundry equipment repair services and parts, as needed for the DuPage Care Center, for the period December 2, 2017 through December 1, 2018, decreasing and closing contract in the amount of $10,416.37, contract 2902-0001 SERV expired.
