A. CS Requests That Also Require Finance And/Or County Board Approval 1. FI-R-0141-17 RESOLUTION -- Contribution to the Northern Illinois Food Bank - Corporate Fund Special Accounts - Company 1000 Accounting Unit 1180 Account 53700 in the amount of $150,000.00 2. HHS-P-0050-17 Recommendation of approval for a contract Issued to Family Shelter Services, Inc., to provide Victim Court Advocacy Services For Victims And Their Children Of Domestic Violence. This contract covers the period of December 1, 2016 through November 30, 2017 for Community Services/Psychological Services, for a contract total amount of $60,000.00; Other Professional Service not subject to competitive bidding per 55 ILCS 5/5-1022(a). Vendor selected pursuant to DuPage County Code Section 2-300.4-108 (1) (b). 

B. CS Requests for Parent Committee Final Approval 1. 2017-36 Recommendation for the approval of a contract purchase order to James P. Corcoran, M.D. to provide medical consultation and paperwork review on substance abuse clients at Psychological Services as required by the Illinois DHS-Division of Alcoholism and Substance Abuse (DASA). This contract covers the period of December 1, 2016 through November 30, 2017 for Community Services, for a contract total amount of $6,200.00. Other Professional Service not subject to competitive bidding per 55 ILCS 5/5-1022(a). Vendor selected pursuant to DuPage County Code Section 2-300.4-108 (1) (b).

2. 2017-37 Recommendation for approval for a contract purchase order issued to Redwood Toxicology Laboratory to provide drug urine testing in court ordered substance abuse treatment. This contract covers the period of December 1, 2016 through November 30, 2017, for Community Services/Psychological Services, for a contract total amount not to exceed $13,000.00, per low quote. 

7. Community services - Mary Keating 

A. CDC Requests That Also Require Finance And/Or County Board Approval HHS-R-0142-17 RESOLUTION -- Recommendation for approval of a Community Development Block Grant (CDBG) Agreement Modification third time extension and first scope of work modification between DuPage County and Family Shelter Service (FSS), Project CD15-11. The third six-month time extension will be until 9/30/2017. Change in scope of work will be from rehabilitation of two shelter facilities to an increase in the number of FSS shelter beds in one facility. 

B. CDC Requests for Parent Committee Final Approval 1. Action Item -- Recommendation for approval of suspension of HOME Program loan payments from Joseph Corp. and Coulter Court Renewal, Inc. for projects HM99-03 and HM02-06, until June 30, 2017 and execution of related loan modification documents. 2. Action Item -- Recommendation for approval of a HOME Investment Partnerships Program Agreement Modification, second time extension between DuPage County and Bridge Communities, Project Number HM15-05 scattered site rehabilitation of 26 rental units in 4 separate properties, extending the agreement through July 31, 2017. 

8. Convalescent Center - Jennifer Ulmer Convo Requests That Also Require Finance And/Or County Board Approval HHS-P-0051-17 Recommendation for the approval of a contract purchase order issued to Advacare Systems, for the rental of medical equipment, low air loss and bariatric mattresses/beds and low air loss wheelchair cushions (category 1 & 2), for the DuPage Convalescent Center, for the period January 26, 2017 through January 25, 2018, for a contract total not to exceed $80,000.00, per lowest responsible bid #17- 004-GV.

9. Budget transfers Budget Transfers -- Community Services - Funds needed to replace current Community Development vehicle, a 2001 Chevy Blazer, with a newer, more environmentally sound vehicle - $25,000.00

10. Travel requests - out of State - required County Board approval 

A. Authorization to Travel -- Community Services Director to attend the National Association for County Community and Economic Development (NACCED) committee meetings and briefings with Housing and Urban Development (HUD) officials in Washington D.C. February 23, 2017, through February 25, 2017. Expenses to include transportation, lodging, and per diem for approximate total of $873.00. Grant funded. 

B. Authorization to Travel -- DuPage Convalescent Center Clinical Staff Educator to attend the Infusion Nurse Society Annual Convention & Industrial Exhibit for re-certification as a Certified Registered Nurse Infusionist in Minneapolis, Minnesota, May 5, 2017 through May 9, 2017. Certification is required to provide annual mandatory training to staff nurses on infusion nursing standards, policies, and procedures. Travel Expenses to include registration, transportation, lodging, and per diem for approximate total of $1,604.20.
