A. Action Item -- Recommendation for approval of a Community Development Block Grant (CDBG) Agreement Modification third time extension and first scope of work modification between DuPage County and Family Shelter Service, Project CD15-11. The third Six-month time extension will be until 9/30/2017. Change in first scope of work will be from rehabilitation of two shelter facilities to an increase in the number of FSS shelter beds by seven in a single facility.

B. Action Item -- Recommendation for approval of a Community Development Block Grant – Disaster Recovery Project Modification Request between DuPage County and the Village of Addison, Project CDBG-DR-13, Property Acquisition.

A. Informational -- Update on Community Development Block Grant – Disaster Recovery - Substantial Amendment #5 and Outreach for Additional Unmet Need of Infrastructure Projects.
