This contract covers the period of December 1, 2017 through November 30, 2018 for Community Services/Psychological Services, for a contract total amount not to exceed $60,000.00; Other Professional Service not subject to competitive bidding per 55 ILCS 5/5-1022(a). Vendor selected pursuant to DuPage County Code Section
2-300.4-108 (1) (b). 2. HHS-P-0007-18 Recommendation for the approval of a contract purchase order to Music Speaks, LLC, for music therapy for the residents of DuPage Care Center, for the period December 12, 2017 through December 11, 2018, for a contract total not to exceed $34,580.00, per most qualified offeror, RFP #17-206-JM.
3. HHS-P-0008-18 Recommendation for the approval of a contract purchase order to McKesson Medical-Surgical, for prime medical supplies, for the DuPage Care Center, for the period January 15, 2018 through January 14, 2019, for a contract total amount not to exceed $260,000.00. Contract pursuant to the Intergovernmental Cooperation Act, MMCAP.
4. Change Order -- HHS-P-0383A-17 Amendment to Resolution HHS-P-0383-17 (County Contract 2772-0001SERV), issued to Appliance, Carpet, Parts LLC, to furnish, deliver, and install energy star rated appliances with proper disposal for Community Services/Weatherization, for the period of July 1, 2017 through June 30, 2018, for the use of additional funds from the Illinois Department of Commerce and Economic Opportunity (ILDCEO), to increase encumbrance in the amount of $18,845.00, resulting in an amended contract total not to exceed $45,965.00 an increase of 69.49%.
D. Judicial/Public Safety - Eckhoff 1. JPS-P-0011-18 Recommendation for the approval of a contract purchase order to Kalkman Investigations & Research Associates, Inc., for services to perform background investigations for prospective deputy sheriff positions. This contract covers the period of December 30, 2017 to December 29, 2018, for the DuPage County Sheriff’s Merit Commission, for a contract total amount not to exceed $33,000.00. Other than lowest responsible bid #17-199-BF.
E. Public Works - Healy
1. FM-P-0005-18 Recommendation for the approval of a contract purchase order to W.W. Grainger, Inc., to furnish and deliver miscellaneous maintenance, repair and operations parts and supplies, as needed for County facilities, for the period January 1, 2018 through December 31, 2018, $100,000.00 for Facilities Management, $60,000.00 for Public Works and $35,000.00 for the Division of Transportation, for a total contract amount not to exceed $195,000.00. Per Intergovernmental Agreement between the County of DuPage and the National Intergovernmental Purchasing Alliance (National IPA)
2. FM-P-0004-18 Recommendation for the approval of a contract purchase order to A&P Grease Trappers, Inc., for sanitary, grease trap and storm basin pumping, jetting and cleaning, as needed for the County campus, for Facilities Management, for the period January 11, 2018 through January 10, 2019, $44,500.00 for Animal Control, $10,100.00 for Division of Transportation, and $56,165.00 for Facilities Management, for a total contract amount not to exceed $70,765.00, per renewal option under bid award #16-224-BF, first option to renew.
3. FM-P-0010-18 Recommendation for the approval of a contract purchase order to Antarctic Mechanical Systems, Inc. d/b/a AMS Mechanical Systems, Inc., to provide time and material plumbing services as needed for County facilities, for the period January 13, 2018 through January 12, 2019, for a total contract amount not to exceed $50,000.00, per renewal option under bid award #15-238, second option to renew.
