A. 2022-8 Recommendation for the approval of the renewal of veterinary pharmaceuticals and medical supplies contract purchase order to Zoetis US LLC, for DuPage County Animal Services for the period December 15, 2021 through December 14, 2022, for a contract total not to exceed $23,000, first of three (3) available options to renew. County Bid Rules waived with initial contract due to the fact that the Zoetis Shelter Program allows for direct purchases from the manufacturer resulting in a significant annual savings of 37% over the next competitively priced reseller.

H. HHS-P-0019-22 Recommendation for the approval of a contract purchase order to Family Shelter Services, Inc., to provide Advocacy Services to Victims of Domestic Violence. This contract covers the period of December 1, 2021 through November 30, 2022 for Community Services, for a contract total amount not to exceed $85,000; Other Professional Service not subject to competitive bidding per 55 ILCS 5/5-1022(a). Vendor selected pursuant to DuPage County Code Section 2-300.4-108 (1) (b).

I. Authorization for Overnight Travel -- Community Services Administrator is a nationally certified Results Oriented Management and Accountability (ROMA) Trainer. The Administrator will attend and teach Roma training at a combined Illinois Department of Commerce and Economic Opportunity (ILDCEO) and IACAA training in Springfield, Illinois from January 11, 2022 through January 13, 2022. Expenses to include miscellaneous expenses (parking, mileage, etc.), lodging, and per diem for a total of $617.00, to be partially or fully reimbursed by the IACAA/ILDCEO.

J. Authorization for Overnight Travel -- Weatherization Assessor to take the Energy Auditor Field Exam in Champaign, Illinois from December 19, 2021 through December 20, 2021. Expenses to include transportation, lodging, and per diems for approximate total of $428.66. Grant funded. (Community Services)

E. FM-P-0021-22 Recommendation for the approval of a contract to Home Depot U.S.A., Inc., for miscellaneous maintenance, repair and operation parts and supplies, as needed, for County facilities, for Facilities Management, for the period January 1, 2022 through December 31, 2026, for a total contract amount not to exceed $150,000. Contract let pursuant to the Intergovernmental Cooperation Act US Communities (OMNIA Partners) #16154.

F. PW-CO-0002-22 PW-CO-0031B-20 - Amendment to Resolution PW-CO-0031A-20, issued to Polydyne, for additional polymer for the Woodridge Greene Valley and Knollwood Facilities, for Public Works, to increase the contract in the amount of $4,622.45, taking the original contract amount of $246,452.85, and resulting in an amended contract total amount not to exceed $251,075.30 an increase of 1.88%

SM-P-0014-22 Recommendation to enter into an Agreement between the County of DuPage Illinois and Engineering Resource Associates, Inc. for Professional Engineering Services to design drainage, stormwater and flood control improvements throughout the County. This contract covers the period December 14, 2021-November 30, 2023 for a contract total not to exceed $200,000. Professional Services in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/ et. Seq vetted through a qualification based selection process (Architects, Engineers and Land Surveyors).

C. SM-P-0015-22 Recommendation to enter into an Agreement between the County of DuPage Illinois and V3 Companies, Ltd. for Professional Engineering Services to design drainage, stormwater and flood control improvements throughout the County. This contract covers the period December 14, 2021-November 30, 2023 for a contract total not to exceed $200,000. Professional Services in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/ et. Seq vetted through a qualification based selection process (Architects, Engineers and Land Surveyors).

D. SM-P-0016-22 Recommendation to enter into an Agreement between the County of DuPage Illinois and Hey & Associates, Inc. for Professional Engineering Services to design streambank stabilization projects throughout the County. This contract covers the period December 14, 2021-November 30, 2022 for a contract total not to exceed $85,000. A. TE-P-0023-22 Recommendation for the approval of a contract purchase order to Accela, Inc. for Managed Application Services for the Building & Zoning, Transportation, Stormwater, and Public Works departments to provide support with permitting software that will allow on-line submittals, electronic document reviews, permit tracking and mobile inspections by applicants and/or County staff, as well as the creation of specifications for and completion of development of four Adjudication reports for Building & Zoning. This contract covers the period of the period February 21, 2022 through February 20, 2023, for a contract total not to exceed $220,200. Exempt from bidding per 55 ILCS 5/5-1022 “Competitive Bids” (c) not suitable for competitive bids – Sole Source. Accela is the sole provider for this software application.

B. TE-P-0024-22 Recommendation for the approval of a contract purchase order to CA Technologies d/b/a Broadcom Inc., to provide E-Server support/diagnostic/resolve software licensing, for Information Technology, for the period of December 15, 2021 through November 30, 2024, for a contract total amount of $184,297, per 55 ILCS 5/5-1022 (c) “Competitive Bids” not suitable for competitive bidding - Sole Source. This is proprietary software.

19. TRANSPORTATION - PUCHALSKI

A. DT-R-0001-22 RESOLUTION -- Adoption of Long Range Transportation Plan

B. Amendment -- DT-R-0400A-21 is an Amendment to resolution DT-R-0400-21, issued to SKC Construction, Inc. for the 2021 Pavement Preservation/Crack Sealing Program Section 21-CRKSL-07-GM, to Increase the funding in the amount of $388.80 PO 5418, resulting in a final County cost of $22,488.80, an Increase of 1.76%.

C. DT-P-0001-22 Recommendation for the approval of a contract to Lakeside International, LLC, to furnish and deliver Navistar/International repair and replacement parts, as needed for the Division of Transportation, for the period January 29, 2022 through January 28, 2024, for a contract total not to exceed $110,000; Per lowest responsible bid 19-168-DOT.

D. DT-P-0002-22 Recommendation for the approval of a contract to Monroe Truck Equipment, Inc., to furnish and deliver one (1) Asphalt Hotbox Reclaimer Unit for the Division of Transportation, for a contract total not to exceed $33,819; Contract pursuant to the Intergovernmental Cooperation Act (NJPA-Sourcewell) # 080521-KMI.
