HHS-P-0436-21 Recommendation for the approval of a contract purchase order to Symbria, Inc., for the Community Wellness Partner for the Wellness and Outpatient Center (staffing and management) at the DuPage Care Center, for the period November 30, 2021 through November 30, 2022, for a contract total not to exceed $65,000, per most qualified offeror, per #21-073-CARE.
5. HHS-P-0437-21 Ratification of an Emergency Procurement to Performance Food Services, for a secondary food supplier, for the DuPage Care Center, cafeteria and cafes on County campus, for the period November 24, 2021 through May 31, 2022, for a total contract amount not to exceed $50,000, per Emergency Procurement per 55 ILCS 5-1022 based upon being necessary to ensure public health and safety.
6. Change Order -- HHS-P-0005A-21 Amendment to Resolution HHS-P-0005-21, issued to Outreach Community Services, Inc., to increase encumbrance in the amount of $15,000, for additional grant funding received by the ILDCEO, resulting in an amended contract total not to exceed $100,000 an increase of 17.65%.
