HHS-P-0326-21 Recommendation for the approval of a contract to Lakeshore Dairy,  Inc., for fluid dairy, for the DuPage Care Center residents, cafeteria and cafes located in the JTK Administration Building and the Judicial Office Facility, for the period  September 1, 2021 through August 31, 2022, for a contract total not to exceed  $80,000.00, per lowest responsible bid #18-124-GV, third and final optional renewal. 

3. HHS-P-0329-21 Ratification of an Emergency Procurement to Alpha Baking Co., for  assorted sliced breads, rolls and buns, for the DuPage Care Center, cafeteria and cafes  on County campus, for the period August 5, 2021 through August 4, 2022, for a total  contract amount not to exceed $50,689.08, per Emergency Procurement per 55 ILCS  5-1022 based upon being necessary to ensure public health and safety. 

4. Change Order -- HHS-P-0105A-21 - Amendment to Resolution HHS-P-0105-21,  issued to Medsearch Staffing Services dba United Pharmacy, for supplemental  staffing for the Pharmacy Department at the DuPage Care Center, for the period May  14, 2021 through May 13, 2022, to increase encumbrance in the amount of $75,000,  resulting in an amended contract total amount not to exceed $125,000, a 150%  increase. E. Judicial/Public Safety - Renehan 

1. JPS-CO-0497-21 Amendment to Resolution JPS-P-0212-20, (purchase order 4803- 0001 SERV), issued to Level 3 Financing, Inc, to provide telecommunication circuits  for disaster recovery and cloud backup for the period October 1, 2020 through  September 30, 2024, to increase encumbrance in the amount of $54,690.72, resulting  in an amended contract total amount not to exceed $274,098.72, in increase of  24.93%. F. Public Works - Ozog  

1. PW-P-0328-21 Recommendation for the approval of a contract to Weatherproofing  Technologies Inc., for roof restoration at various Public Works Facilities, for the  period August 10, 2021 through June 1, 2022 for a total contract amount not to  exceed $250,648.51. Contract pursuant to the Intergovernmental Cooperation Act – Omnia Partners #R180903. 

2. PW-R-0355-21 RESOLUTION -- DuPage River Salt Creek Workgroup Membership  Dues for 2021-2022 are in the amount of $279,825, to be cost shared in the amount of  $34,147.68 to be paid by Stormwater Management and $245,677.32 to be paid by  Public Works. Public Works dues include a special condition fee for NPDES permits  per IEPA/EPA. 

G. Technology - Selmon  

1. TE-P-0322-21 Recommendation for the approval of a contract to SHI International  Corp., for the procurement of 200 laptops and 230 docking stations for County  employees, for Information Technology, for a contract total amount of $258,398.90,  per lowest responsible bid #21-063-IT. An additional 30 docking stations were added  to this procurement via quote #20702773 using the same bid price to support  previously purchased laptops. 

H. Transportation - Puchalski 

1. Change Order -- Recommendation for the approval of the Amendment to Resolution  DT-P-0270-19, issued to Ciorba Group, for the 2019 bridge inspection contract,  Section 19-BRDGE-07-EG, to increase the contract in the amount of $40,000,  resulting in an amended contract total amount not to exceed $135,000, an increase of  42.11% and to extend the contract until June 30, 2022.
