B. HHS-R-0266-21 RESOLUTION -- Approval of the DuPage County Ride DuPage  Program Title VI Program Notice

HHS-P-0104-21 Recommendation for the approval of a contract purchase order to  McKesson Medical-Surgical Government Solutions LLC, to furnish and deliver  incontinent products, for the DuPage Care Center, for the period June 1, 2021 through  May 31, 2022, for a total contract amount not to exceed $195,000.00, per bid renewal  under bid #20-025-CARE, first of three (3) one (1) year optional renewals. 

D. HHS-P-0105-21 Recommendation for the approval of a contract to United Pharmacy  Staffing, for supplemental staffing for the Pharmacy Department at the DuPage Care  Center, for the period May 14, 2021 through May 13, 2022, for a contract total not to  exceed $50,000.00, per quote #20-144-CARE. 

E. HHS-P-0108-21 Recommendation for the approval of a contract to AmerisourceBergen  Drug Corporation, for the purchase of pharmaceuticals for the inpatient and outpatient  Pharmacy Services at the DuPage Care Center, for the period June 25, 2021 through June  24, 2022, for a contract total amount not to exceed $2,400,000.00, per renewal option  under bid #18-109-DT, third and final optional renewal. 

F. HHS-P-0109-21 Recommendation for the approval of a contract purchase order to Sysco  Chicago, Inc., for primary food supplies and chemicals for the DuPage Care Center  Dining Services and JTK Administration and Judicial Office Facility campus cafes for  the period June 1, 2021 through May 31, 2022, for a total amount not to exceed  $1,152,570.00, per renewal under Proposal #P18-045-GV, third and final optional  renewal.

A. JPS-P-0103-21 Recommendation for the approval of a renewal contract purchase order to  HOV Services, to provide professional consultation and technical assistance with  application systems. This contract covers the period of June 1, 2021 through May 31,  2022 for the Circuit Court Clerk, for a contract total amount not to exceed $723,699.37.  Renewal from purchase order 4623-SERV (RFP 20-029-CRT). A. PW-R-0267-21 RESOLUTION -- Recommendation for approval to grant signature  authority to the Director of Public Works, for amounts less than $15,000, to acquire  necessary property interests for Public Works projects. 

B. FM-P-0107-21 Recommendation for the approval of a contract to C. Armbrust Plumbing  & Heating, Inc., to provide time and material plumbing services as needed, for County  facilities, for the period May 15, 2021 through May 14, 2022, for Facilities Management,  for a contract total amount not to exceed $50,000, per renewal option under bid award 

#19-039-FM, second option to renew. 

C. PW-P-0106-21 Recommendation for the approval of a contract to Visu-Sewer of Illinois,  LLC for CIPP sanitary sewer lining, for Public Works, for the period May 11, 2021  through November 30, 2021, for a total amount not to exceed $241,780, per lowest  responsible bid #21-026-PW

A. SM-R-0259-21 RESOLUTION -- Recommendation for the approval to enter into an  Intergovernmental Agreement between the County of DuPage, Illinois and the Wheaton  Sanitary District for the Spring Brook #1 Streambank Stabilization Project, for an  agreement not to exceed $54,099.34. 

B. SM-R-0260-21 RESOLUTION -- Recommendation for the approval to enter into an  Intergovernmental Agreement between the County of DuPage, Illinois and the Carol  Stream Park District for the Armstrong Park North Parking Lot Permeable Paver Project,  for an agreement not to exceed $40,395.67. 

C. SM-R-0262-21 RESOLUTION -- Recommendation for approval of Modification One to  the Interdepartmental Memorandum of Understanding for HUD Funding of the Elmhurst  Quarry Sluice Gate Installation Project. 

D. SM-R-0265-21 RESOLUTION -- Recommendation for Acceptance of a Temporary  Construction Easement - Stanek - $1.00. This Easement is for a small scale drainage  project to reduce localized flooding in unincorporated Lombard. 

E. SM-R-0263-21 RESOLUTION -- Recommendation for Acceptance of a Temporary  Construction Easement - Caraballo - $1.00. This Easement is for a small scale drainage  project to reduce localized flooding in unincorporated Glen Ellyn. 

F. SM-R-0264-21 RESOLUTION -- Recommendation for Acceptance of a Permanent  Drainage Easement - Stark - $1.00. This Easement is for a small scale drainage project to  reduce localized flooding in unincorporated Glen Ellyn.

G. SM-R-0261-21 RESOLUTION -- Recommendation for approval to grant signature  authority to the Director of Stormwater Management, for amounts less than $15,000, to  acquire necessary property interests for Stormwater Management projects. 

H. SM-P-0100-21 Recommendation to enter into an Agreement with Alfred Benesch &  Company, for Professional Land Surveying Services, for Stormwater Management. This  contract covers the period through April 30, 2022, for a contract total amount not to  exceed $30,000.00. Professional Services in compliance with the Illinois Local  Government Professional Services Selection Act, 50 ILCS 510/ et. seq. Vetted Through a  qualification based selection process (Architects, Engineers and Land Surveyors). 

I. SM-P-0101-21 Recommendation for the approval of a contract issued to WBK  Engineering, LLC, for On Call Professional Engineering Services, for Stormwater  Management. This contract covers the period through April 30, 2022, for a contract total  not to exceed $75,000.00. Professional Services (Architects, Engineers and Land  Surveyors) vetted through a qualification based selection process in compliance with the  Illinois Local Government Professional Services Selection Act, 50 ILCS 510/et. seq. 

J. SM-P-0102-21 Recommendation for the approval of a contract to Runnion Equipment  Company, for crane truck repairs, as needed for the Stormwater Management  Department, for the period May 4, 2021 through November 30, 2021, for a contract total  not to exceed $52,634.51; Per 55 ILCS 5/5-1022(c) “not suitable to competitive bids”  (Direct replacement of compatible equipment parts). 

15. TRANSPORTATION - PUCHALSKI 

Committee Update 

A. DT-R-0268-21 RESOLUTION -- Intergovernmental Agreement between the County of  DuPage and Milton Township Road District, for the 2021 Road Maintenance Program,  Section 21-05000-01-GM, for an estimated Township cost of $640,000. (No County cost) 

B. DT-P-0110-21 Recommendation for the approval of a contract to Northern Contracting,  Inc., to furnish, deliver, repair and install guardrails, as needed for the Division of  Transportation, for the period May 12, 2021 through May 11, 2022, for a contract total  not to exceed $100,000.00; Per lowest responsible bid 21-028-DOT. 

C. DT-P-0111-21 Recommendation for the approval of a contract to BCR Automotive  Group, LLC, d/b/a Roesch Ford, to furnish and deliver Ford/Motorcraft repair and  replacement parts, as needed for the Division of Transportation, for the period May 13,  2021 through May 12, 2022, for a contract total not to exceed $70,000; Per renewal  option under bid award 20-028-DOT, first of three options to renew. 

D. DT-P-0112-21 Recommendation for the approval of a contract to Utility Transport  Service, Inc., for solid waste disposal of street sweepings and storm sewer and ditching  spoils, as needed for the Division of Transportation, for the period June 1, 2021 through  May 31, 2022, for a contract total not to exceed $60,000; Per renewal option under bid  award 20-040-DOT, first of three options to renew.

E. Amendment -- DT-P-0239B-20 - Amendment to Resolution DT-P-0239A-20 issued to  BLA, Inc., to provide Professional Construction Engineering Services for improvements  along CH 34/31st Street, from Meyers Road to York Road, Section 14-00259-05-CH;  (Correction of scrivener’s error). 

F. Change Order -- DT-P-0119A-20 - Amendment to Resolution DT-P-0119-20, issued to  Precise MRM, LLC, for fleet tracking equipment and service agreement, for the Division  of Transportation, to increase the funding in the amount of $6,705, resulting in an  amended contract total amount of $49,495, an increase of 15.67%.
