West Suburban Nursing and Rehab Center, a 259-bed skilled care facility in Bloomingdale, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
West Suburban Nursing and Rehab Center was cited with an A violation in November and fined $25,000.
