This contract covers the period of July 18, 2018 through July 17, 2019 for Human Resources, for a contract total amount of $25,759.00, per low bid 18-142-JM.

       O. FI-P-0204-18 Recommendation for the approval of a contract purchase order to Canon Solutions America, Inc., for Multifunctional Device Equipment, Supplies, Software and Service Solutions for all County Departments, covering the period of October 1, 2018 through September 30, 2023, for a contract total amount not to exceed $1,434,498, RFP 18-020-LG, Contract pursuant to the Intergovernmental Cooperation Act (National IPA)

       P. FI-P-0206-18 Recommendation for the approval of a contract purchase order to enter into an agreement with Mesirow Financial Investment Management Inc., to provide 457 plan related services, including strategic plan design, consolidation, and fiduciary services for Finance. This contract covers the period of August 1, 2018 to July 31, 2021, for a contract total amount not to exceed $162,000.00. Per Proposal 18-017-BF.

       A. HHS-R-0242-18 RESOLUTION -- Recommendation for Approval of the Transfer and Reclassification of Eight Liens From Franciscan Ministries to Mercy Housing, for $175,693 in Rental Rehabilitation Program (RRP) for Eight Properties and Sixty-five Units.

       B. HHS-P-0197-18 Recommendations for the approval of a contract purchase order to Joerns LLC, for fluid immersion simulation system (approved equal) for the DuPage Care Center, for the period of August 9, 2018 through August 8, 2019, for a total contract amount of $26,400.00, per low bid #17-110-GV, first of three one-year optional renewals.

       C. Authorization for Overnight Travel -- Community Services Manager to attend the National Adult Protective Services (APS) Conference in Anaheim, California from August 25, 2018 through August 30, 2018. Expenses to include registration, transportation, lodging, and per diem for approximate total of $1,918.00. Seniors grant funded 5000-1720.

       D. Authorization for Overnight Travel -- Family Center Administrator to work with Access & Visitation grant administrators on an education initiative in Springfield, Illinois from July 11, 2018 through July 12, 2018. All costs will be covered by the state department, Healthcare and Family Services; no County costs will be incurred. This travel request will be pre-approved at the Health and Human Services Committee on July 10, 2018; will be presented at County Board as informational only (after the travel) on July 17, 2018.

       E. Authorization for Overnight Travel -- Family Center Program Manager to work with Access & Visitation grant administrators on an education initiative in Springfield, Illinois from July 11, 2018 through July 12, 2018.        B. FM-P-0201-18 Recommendation for the approval of a contract purchase order to Allied Waterproofing, Inc., to provide all labor and materials necessary for the 509 parking structure joint sealant repair, for Facilities Management, for the period July 18, 2018 through November 30, 2018, for a total contract amount not to exceed $377,200.00, per lowest responsible bid #18-108-DT

       C. FM-P-0202-18 Recommendation for the approval of a contract purchase order to Builders Chicago Corporation, to provide preventive maintenance, service and repairs for overhead doors, roll-up shutters, gate operators, dock levelers and revolving doors, as needed for County facilities, for the period August 15, 2018 through August 14, 2019, for Facilities Management, for a contract total amount not to exceed $105,871.00, per renewal option under bid award #16-134-BF, second option to renew.        I. DT-P-0196-18 Recommendation for the approval of a contract purchase order to Genuine Parts Company, d/b/a NAPA Auto Parts, to furnish and deliver automotive repair and replacement parts, as needed for the Division of Transportation, for the period July 21, 2018 through July 20, 2019, for a contract total not to exceed $125,000.00; Contract pursuant to the Intergovernmental Cooperation Act [NASPO]

       J. Change Order -- DT-P-0077A-18 – Amendment to Resolution DT-P-0077-18, issued to DuPage Materials Company, LLC, to furnish and deliver bituminous paving materials, as needed for the Division of Transportation, to increase the funding in the amount of $13,000.00, resulting in an amended contract total amount of $43,000.00, an increase of 43.33%

       K. Change Order -- DT-P-0079A-18 – Amendment to Resolution DT-P-0079-18, issued to DuKane Asphalt Company, to furnish and deliver bituminous paving materials, as needed for the Division of Transportation, to increase the funding in the amount of $30,000.00, resulting in an amended contract total amount of $57,000.00, an increase of 90.0%

Action Item -- DT-R-0232B-14 – Amendment to Resolution DT-R-0232A-14, Local Agency Agreement between the County of DuPage and the Illinois Department of Transportation, for intersection improvements at CH 21/Fabyan Parkway and Illinois Route 38, Section 11-00210-04-CH, to decrease the funding in the amount of $306,000.00, resulting in an amended total amount of $1,592,660.00, a decrease of 16.12%

       M. Action Item -- DT-R-0186A-17 – Amendment to Resolution DT-R-0186-17, Local Public Agency Agreement between the County of DuPage and the Illinois Department of Transportation, for improvements along CH 43/County Farm Road, from US Route 20 to Schick Road, Section 14-00179-31-SP, to decrease the funding in the amount of $200,000.00, resulting in an amended total amount of $481,845.00, a decrease of 29.33%
