Bensenville School District 2 recentlyissued the following announcement.
For the purpose of safeguarding the health of children, District 2 enforces Illinois health requirements that consist of health, dental, and eye exams, and immunizations specific to grade and age.
Students who do not provide the required immunization and/or exam records will be excluded from school according to District policy and Illinois School Code. (Click here for Board Policy 7:100 Health Examinations, Immunizations, and Exclusion of Students.)
Physical examinations are required for all children entering pre-kindergarten, kindergarten, and sixth grade, and those who are new to the school. Bring your child’s completed State of Illinois Certificate of Child Health Examination Form to your school’s health office by the first day of school.

This exam form must be dated within one year prior to the first day of school. It must also be completed and signed by a licensed healthcare provider, and it must include a diabetes screening and lead screening for children who are 6 years old or younger. A tuberculosis (TB) skin test is recommended.
The Health History section of the form must be completed and signed by the parent/guardian.
IMMUNIZATION REQUIREMENTS
Students are required to show proof of immunizations, including the month/date/year. Failure to provide the information will result in the child’s exclusion from school.
Pre-Kindergarten
- DPT (Diphtheria, Pertussis, Tetanus) – 4 doses at appropriate intervals
- Polio -- 3 doses
- Hib (Haemophilus Influenzae Type B) – 1 dose
- MMR(Measles, Mumps, Rubella) – 1 dose
- Varicella (Chickenpox) – 1 dose after 12 months
- Hepatitis B Series – 3 doses
- PCV13 (Pneumococcal Conjugate Vaccine) – Primary series or proof of 1 dose
- DPT – 3 doses at appropriate intervals
- Polio – 4 doses at appropriate intervals
- MMR – 2 doses
- Varicella – 2 doses
- DPT – 3 doses at appropriate intervals
- Polio – 3 doses at appropriate intervals
- MMR – 2 doses
- Varicella – 2 doses
- DPT – 3 doses at appropriate intervals
- Polio – 3 doses at appropriate intervals
- MMR – 2 doses
- Varicella – 2 doses
- Tdap – 1 dose since last DTAP, DT, or Td dose
- Polio – 3 doses at appropriate intervals
- MMR – 2 doses
- Varicella – 2 doses after 12 months of age

- MCV4 (Meningococcal Conjugate) – 1 dose
