This contract covers the period of December 1, 2022 to December 1, 2023 for Finance, for a contract total amount not to exceed $358,281. Vendor selected pursuant to DuPage County Code Section 2-353(1)(b).

I. FI-P-0315-22 Recommendation for a contract purchase order to Mesirow Insurance Services, Inc. (Safety National), to provide Workers' Compensation Insurance to the County and the Health Department. This contract covers the period of December 1, 2022 to December 1, 2023 for Finance-Tort Liability, for a contract total amount not to exceed $172,321. Vendor selected pursuant to DuPage County Code Section 2-353(1)(b).

J. FI-P-0316-22 Recommendation for a contract purchase order to Mesirow Insurance Services, Inc. (Freedom Specialty), to provide Employment Practices Liability Insurance for Finance-Tort Liability. This contract covers the period of December 1, 2022 to December 1, 2023, for a contract total amount not to exceed $52,708. Vendor selected pursuant to DuPage County Code Section 2-353(1)(b).

K. FI-P-0317-22 Recommendation for approval of a contract purchase order to Mesirow Insurance Services, Inc. (Safety National/APR/Kinsale/AWAC), for excess General and Automobile Liability Insurance for the County and the Health Department. This contract covers the period December 1, 2022 to December 1, 2023 for Finance-Tort Liability, for a contract total amount not to exceed $687,128. Vendor selected pursuant to DuPage County Code Section 2-353(1)(b).

L. Change Order -- Amendment to Purchase Order 5835-0001 SERV, issued to ODP Business Solutions, to increase the Purchase Order in the amount of $14,000, to cover the cost of office supplies for various County departments due to increased pricing, for a new Purchase Order amount not to exceed $47,275.45.

Design work will cover a modernization and upgrade of the Kenneth Moy DuPage Care Center facility, for the period November 9, 2022 through December 31, 2026, for a total contract amount not to exceed $1,806,300. C. J. SM-P-0309-22 Recommendation to enter into an Agreement between the County of DuPage, Illinois and V3 Companies LTD., for Professional Engineering Services for the development of the Lacey Creek Watershed Plan, for Stormwater Management. This contract covers the period November 8, 2022 through November 30, 2024, for a contract total not to exceed $91,788.80. Professional Services in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/ et. seq. vetted through a qualification based selection process (Architects, Engineers and Land Surveyors).

K. SM-P-0310-22 Recommendation to enter into an Agreement between the County of DuPage, Illinois and Cemcon, Ltd., for Professional Engineering Services for hydraulic modeling and floodplain mapping services, for Stormwater Management. This contract covers the period December 1, 2022 through June 30, 2024, for a contract total not to exceed $95,000. Professional Services in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/ et. seq. vetted through a qualification based selection process (Architects, Engineers and Land Surveyors).

L. SM-P-0311-22 Recommendation to enter into a Joint Funding Agreement between The County of DuPage, Illinois and the United States Department of the Interior - U.S. Geological Survey (USGS), for Water Resources Investigations, for the period of December 1, 2022 through November 30, 2023, for Stormwater Management, for a contract total amount of $367,000; USGS shall contribute $180,700.

TE-P-0312-22 Recommendation for the approval of a contract purchase order to Imaging Systems, Inc. d/b/a Integrated Document Technologies (IDT), for the annual Hyland and CAPSYS Software Assurance maintenance of imaging systems for various departments, for Information Technology. This contract covers the period of January 1, 2023 through January 31, 2024, for a contract total amount of $58,227.56. Exempt from bidding per 55 ILCS 5/5-1022 (c) “Competitive Bids” not suitable for competitive bids – Sole Source. IDT is the OnBase approved provider for their products.

B. TE-P-0313-22 Recommendation for the approval of a contract purchase order to IBM Corporation, for the purchase of program product software licenses for IBM z/OS and software support and maintenance for the BC12 z System Server, for Information Technology. This contract covers the period of December 1, 2022 through November 30, 2023, for a total contract amount of $111,171.72. Exempt from bidding per DuPage County Purchasing Ordinance, Article 2-350 - Sole Source. This is proprietary software.

A. DT-R-0461-22 RESOLUTION -- Agreement between the County of DuPage and the Chicago Metropolitan Agency for Planning (CMAP), for Safe Streets and Roads for All Grant, (County cost $60,000.00).

B. DT-P-0300-22 Recommendation for the approval of a contract to S.T.A.T.E. Testing, LLC, for Professional Materials Testing and Engineering Services, upon request of the Division of Transportation ($84,000), Facilities Management ($8,000), and Stormwater Management ($8,000), for a contract total not to exceed $100,000; Professional Services (Architects, Engineers and Land Surveyors) vetted through a qualification based selection process in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/0.01 et. seq.

C. DT-P-0308-22 Recommendation for the approval of a contract to Huff & Huff, Inc., to provide Professional Landscape Design and Engineering Services for various County highway projects, Section 22-LDSCP-06-EG, for a contract total not to exceed $100,000; Professional Services (Architects, Engineers and Land Surveyors) vetted through a qualification based selection process in compliance with the Illinois Local Government Professional Services Selection Act, 50 ILCS 510/0.01 et. seq.

D. Amendment -- Recommendation of Approval to Amend ODT-00001A-06, DuPage County Highway Rights-Of-Way Permit and Fee Ordinance.

E. Amendment -- DT-R-0039A-22 – Amendment to Resolution DT-R-0039-22, Local Agency Agreement between the County of DuPage and the Illinois Department of Transportation, for improvements to CH 2/Hobson Road/63rd Street from Woodridge Drive to Janes Avenue, Section 16-00146-00-SW, to increase the County’s share of funding in the amount of $600,000, resulting in an estimated County cost of $900,000, an increase of 200.00%

F. Amendment -- DT-P-0242B-19 – Amendment to Resolution DT-P-0242A-19, issued to TranSystems Corporation, for Professional Preliminary Engineering Services for improvements at CH 23/ Naperville Road at Illinois 38, Section 19-00195-05-CH, to increase the funding in the amount of $43,641.55, resulting in an amended contract total amount of $329,559.09, an increase of 15.26%.

G. Amendment -- DT-P-0155A-22-Amendment to Resolution DT-P-0155-22, issued to Patrick Engineering for Professional Phase I Preliminary Engineering and Phase II Design Engineering Services for improvements to the bridge carrying CH 21/Geneva Road over the West Branch of the DuPage River, Section 18-00206-10-BR, to increase the funding in the amount of $162,454.55, resulting in an amended contract total of $694,378.58, an increase of 30.54%.

H. Amendment -- DT-R-0173A-22 – Amendment to Resolution DT-R-0173-22, issued to R.W. Dunteman Company for the 2022 Pavement Maintenance (North) Program, Section 22-PVMTC-17-GM, to increase the funding in the amount of $225,000.00, resulting in a final County cost of $5,613,504.00, an increase of 4.18%

I. Amendment -- DT-R-0172B-22 – Amendment to Resolution DT-R-0172A-22, issued to K-Five Construction Corporation for the 2022 Pavement Maintenance (South) Program, Section 22-PVMTC-18-GM, to increase the funding in the amount of $100,000, resulting in an amended contract total of $4,797,388.59, an increase of 2.13%.

J. Amendment -- DT-R-0004B-20 – Amendment to Resolution DT-R-0004A-20, Local Agency Agreement between the County of DuPage and the Illinois Department of Transportation, for improvements to CH 33/75th Street, from Lyman Avenue to Exner/Williams Road, Section 16-00233-00-CH, to increase the County’s share of funding in the amount of $310,000, resulting in an estimated final County cost of $1,340,960, an increase of 30.06%.

K. Amendment -- DT-P-0140A-22 - Amendment to Resolution DT-P-0140-22, issued to HW Lochner, Inc., for Professional Structural Engineering Services for Section 22- STENG-05-EG, to increase the funding in the amount of $245,000, resulting in an amended contract total amount of $595,000, an increase of 70.00%.

L. Amendment -- DT-R-0242A-21 – Amendment to Resolution DT-R-0242-21, issued to H & H Electric Company for mast arm replacement and traffic signal modification at various intersections within DuPage County, Section 21-TSMAR-02-GM, to increase the funding in the amount of $11,500.00, resulting in a County cost of $623,603.55, an increase of 1.88%.

M. Amendment -- DT-R-0284B-17 – Amendment to Resolution DT-R-0284A-17, Intergovernmental Agreement between the County of DuPage and the City of Wheaton, for intersection improvements along CH 23/Gary Avenue at Jewell Road, Section 17- 00237-14-MS, to decrease the funding in the amount of $19,460.23, resulting in an amended estimated County cost of $18,115.01, a decrease of -34.31%.
