Addison School District 4 recently issued the following announcement.
At Governor Pritzker's press conference announced that Illinois will enter Phase 4 on Friday, June 26, Dr. Ezike mentioned that Illinois residents make appointments to visit their doctors now. We would like to remind parents that the Illinois School Code requires that all students entering preschool, kindergarten, and 6th grade receive physicals and the required immunizations before the school year begins. This is also for students who are new to our ASD4 district. Children who do not turn in their immunizations and physicals to school before the first day of school will be excluded from school until these records are up to date.
Additionally, a permission card is required of all students to participate in the Indian Trail School sports program.
