City of West Chicago community leaders have issued a Request for Qualifications (RFQ) for the purposes of identifying and hiring a director for the Healthy West Chicago Community Project.

To that end, the core group of leaders has created an Advisory Board under nonprofit fiscal agent People Made Visible Inc.

The job description of the program’s director includes facilitating implementation of the Healthy West Chicago Community Project (HWC) Action Plan, as well as accurately measuring the plan's results. The overall goal of the HWC Action Plan is to motivate children and families to make changes toward healthier lifestyles by making smarter, healthier choices with meals, exercise and other activities.

The director's role will include driving all HWC programs, measuring the impact of the programs within the community, and working to develop and maintain strong relationships with residents and other stakeholders. The director must emphasize and bolster West Chicago’s community health initiatives by engaging internal and external partners in collaborative interventions and programming, while educating themselves to ensure compliance with relevant law and policy. The person in this position must provide support for health activities at local schools, health systems and all relevant community organizations.

The director will report directly to the Advisory Board to maintain consistency and ensure that community health initiatives remain aligned with identified community health needs. Qualifications must be received by the RFQ Committee no later than 4 p.m. on Jan. 15, 2016. Late qualifications will not be considered.
