Iona Glos Specialized Living Center, a 100-bed intermediate and developmentally disabled care center in Addison, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the first quarter of 2019.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Iona Glos Specialized Living Center was cited with two A violations for incidents in November 2018 and January, and fined a total of $25,000.
