BLOOMINGDALE PARK DISTRICT: Bloomingdale Park District to hold vaccine clinic for 12- to 16-year-olds

Bloomingdale Park District issued the following announcement on May 12.

Bloomingdale Park District, in partnership with Rana-Reagan Pharmacy, is holding a COVID-19 Vaccine Clinic for 12- to 16-year-olds with the first dose of the Pfizer vaccine being given this Friday, May 14 at the Johnston Recreation Center, 172 S. Circle Ave., in Bloomingdale.

Five-hundred spots are available. Register online at http://bit.ly/bpd-vaccine-12-16. Participants must fill out the consent form at http://bit.ly/bpd-vaccine-consent-form and bring it along.

Second dose appointments will be on June 4 at the same time and place as the first dose.

Registration is for anyone 12 years of age or older (provided the CDC approves this age group). Anyone ages 12 to 16 must be accompanied by a parent or legal guardian. Consent forms must be signed by a parent or legal guardian as well. For verification purposes, a valid photo ID with an address and DOB is required.

Please note that appointments are required, and walk-up registration is not available. You will be turned away if you do not have an appointment. Telephone registration is not available for this vaccination event.

Please wear a short-sleeved shirt. Arrive no earlier than 15 minutes before your scheduled appointment.

Please bring a copy of your prescription insurance card (if you have one), Medicare or Medicaid, as well as the completed Consent Form linked above. Insurance is not required to receive a vaccine.

Masks are required to be worn at all times during the clinic, and social distancing will be observed.
